New Phase Project delivers a comprehensive, evidence-informed program built on our PHASE Model—a structured framework designed to create meaningful, long-term behavioral change. Our approach combines in-person programming with telehealth follow-up, providing a true continuum of care that supports participants beyond the classroom.

Continuum of Care

Our program extends beyond initial participation. Through structured telehealth follow-up and ongoing support, we help individuals apply what they’ve learned in real-life situations—reinforcing progress, preventing relapse, and promoting long-term success.
